Our MISSION

SCORES Reentty, Ine has been servicing the State of New Jersey and those impacted by incarceration's and addiction since 2016. With a combined 30 years of experience, SCORES Reentry leadership has worked within urban neighborhoods, with adults and youth impacted by incarceration and addiction, utilizing a social justice and human rights framework. Our team comprises of professionals who have extensive substance abuse treatment experience and are Licensed Drug and Alcohol Counselors and Licensed Social Workers. Additionally, our team also has experience working with individuals who have sexual offense history, severe mental health and geriatric issues.

Services include case management and linkages to housing, employment, supportive services and specialized care. As a bilingual team, the agency has worked with individuals impacted by immigration/ICE detention. Due the agency's ability to be remote and actively engaged in the environment, SCORES Reentry, Inc. services clients residing throughout the State of New Jersey. SCORES Reentry is a State of New Jersey, Certified Equal Rights Opportunity and Affirmative Action Employer (Certification #: 647527). Agency leadership is female and minority led. SCORES Reentry is committed to diversity and inclusion, as evidenced by aligning with partnerships committed to minority owned businesses and the LGBTQA+ communities.
Additionally, the agency has worked with organizations to raise awareness to the promotion of diversity and inclusion within the communities. The agency is also bilingual and services the Latinx communities.

Who are we

SCORES Reentry was created to address systematic barriers impacting those with incarceration history. With our own individual lived experiences, we understand firsthand the impact that incarceration and addiction has on oneself and family. We provide genuine services, strengthening self esteem and confidence through weekly case management services. Our aim is to help as many individuals and their families as possible, stunt the vicious cycle of poverty and incarceration and streamline consistent services to reduce recidivism.

The Focus of Our Efforts
Advanced Discharge Planning Program

The program is offering to provide specialized discharge planning for clients who are in hospitals, long term residential care, halfway houses and incarceration. The focus is to address barriers to healthy discharge planning early in the recovery process by teaming with assigned case manages. This program is the pipeline to comprehensive discharge planning that includes housing stabilization, linkages to specialized services such as credit restoration, financial literacy, criminal expungements, assist with successful job searches, and maintenance of mental health and/or addiction support. Through the teaming of informal and formal supports, this program utilizes the evidence based collaborative care model. This model has been piloted at a long term residential facility and has been proven to be effective in reducing the risk of recidivism. The program is an access point to long term stabilization efforts, utilizing community partnerships. With the support of therapeutic interventions, this discharge planning offers individual, family and group therapy for involved participants. Program is mobile and able to meet the logistical needs of clients
